What Is an NFPA 80 Fire Door Inspection?
An NFPA 80 fire door inspection is a formal, documented examination of a fire-rated door assembly -- the door, frame, hardware, glazing, and seals -- conducted at least once per year. The inspection verifies that the assembly can still perform its rated function: containing fire and smoke long enough to allow egress and limit spread. When NFPA 101 (Life Safety Code) or the International Fire Code is enforced in an adopted edition that references the annual inspection requirement, it is not optional. Deficiencies identified during inspection must be corrected promptly -- in healthcare settings, The Joint Commission treats excessive gaps and inoperative closers as conditions requiring immediate repair.
The 13 Inspection Points (NFPA 80)
The standard identifies the following items for swinging fire door assemblies with builders hardware. Walk each door with this list:
- Labels visible and legible. The fire label on the door and frame must be readable. A painted-over or missing label is a deficiency.
- No open holes or breaks in door or frame surfaces. Any penetration -- an old hardware prep, an abandoned wire hole, a kick-through -- compromises the rated assembly.
- Glazing, vision light frames, and beads intact. If the door has a lite, the glass must be listed fire-rated glazing, and the frame and retaining beads must be secure.
- Door, frame, hinges, hardware, and threshold secured and aligned. Loose hinges, a racked frame, or a missing non-combustible threshold are all deficiencies. Hardware must show no visible damage.
- No missing or broken parts. A broken closer arm, a stripped hinge, a missing latch component -- any absent part fails this point.
- Door clearances within NFPA 80 limits. Gaps matter precisely. Allowable clearances per NFPA 80 are:
- Head and jamb (hollow metal): 1/8 inch plus or minus 1/16 inch
- Head, jamb, and meeting edge (wood fire doors): up to 1/8 inch (up to 3/16 inch on 20-minute assemblies)
- Door bottom: up to 3/4 inch
- Self-closing device operational. The closer must fully close and latch the door from the full-open position on every cycle. A door that drifts open, stops short, or requires a push to latch fails here. NFPA 80 Section 6.4.1 requires a closing device on every fire door.
- Coordinator function (paired doors). On rated pairs, the inactive leaf must close and latch before the active leaf. If the sequence reverses, the inactive leaf blocks the active leaf latch and the pair fails to secure.
- Latching hardware operates and secures the door. The latchbolt or fire latch must positively engage the strike every time the door closes. Positive latching is a non-negotiable NFPA 80 requirement -- a door that rests against the stop but does not latch is not a functioning fire door.
- No auxiliary hardware that interferes with or prohibits operation. Aftermarket door stops wedged under the door, surface-applied bolts that prevent closing, or any device that holds the door open without a listed smoke-detector release are deficiencies. Fusible-link hold-open arms also do not meet NFPA 101 or IBC requirements because they do not respond to smoke.
- No field modifications that void the label. Cutting an unapproved louver, installing an oversized lite, or routing hardware preps not covered by the label engineering all void the assembly. If a modification is needed, it must be performed under a label service program by an authorized agency.
- Meeting edge protection, gasketing, and edge seals present and intact. Where intumescent or smoke-and-draft seals are required, they must be physically present, undamaged, and correctly positioned. A seal that has been painted over, compressed flat, or torn does not perform its listed function.
- Required signage in place. NFPA 80 Section 4.1.4 requires that fire doors carry signage such as "FIRE DOOR -- KEEP CLOSED." Missing or removed signs are a citation.
The Hardware Failures That Fail the Most Inspections
Field experience and industry data consistently point to the same repeat offenders:
- Inoperative or undersized closers. A closer adjusted too light to overcome weatherstripping resistance, or a spring-size mismatch for the door width and weight, is the single most common deficiency. Closers from Hager, Norton, Corbin Russwin, and Accentra (formerly Yale) are available in a full range of spring sizes and are listed for labeled-door applications. Selecting the correct closer at spec time -- not the smallest size that will fit -- prevents this failure.
- Excessive perimeter gaps. Doors sag over time; frames shift; repeated use deforms stops. Once the head or jamb gap exceeds 3/16 inch on a hollow metal assembly, the door is out of tolerance. Hinge shimming (steel fire-rated hinge shims are available), strike shims, and listed gap-filler systems address this without a full door replacement.
- Positive-latch failures. A worn latchbolt, misaligned strike, or missing coordinator on a pair will all prevent positive latching. Exit devices from Sargent, Corbin Russwin, or Hager with properly adjusted latch projection and a correctly shimmed strike restore engagement. Note that flush bolts on the inactive leaf of a rated pair must themselves be listed for fire door use.
- Open hardware preps. When hardware is removed -- a lockset pulled during a renovation, a closer replaced -- the old screw holes and prep cutouts remain. NFPA 80 requires that open holes in fire doors be filled with listed steel fasteners. - Damaged or absent seals. Smoke and draft seals on S-labeled assemblies must be intact and correctly placed on the frame stop at head and both jambs. UL-listed self-adhesive silicone seals, such as NGP 5050 series products, are rated for positive-pressure fire door applications and install without tools beyond a clean surface and firm pressure.
Who Performs the Inspection -- and What Documentation Is Required
NFPA 80 requires that the person performing the inspection be "qualified" -- meaning knowledgeable in fire door assembly requirements. In practice, this means a certified door inspector (such as an Architectural Hardware Consultant or DHI-credentialed inspector), a trained facilities professional, or a qualified contractor. The inspection must be documented, and records must be available to the authority having jurisdiction (AHJ). For healthcare facilities, Joint Commission surveyors routinely request these records during triennial surveys.
